A commonwealth government initiative where people with a chronic medical condition may be able to receive Medicare benefits towards the cost of certain services to help manage their condition.

  • WHAT IS A CHRONIC DISEASE MANAGEMENT PLAN?
    • This was formerly known as the Enhanced Primary Care (EPC) Program
    • This plan requires a referral from your GP who will determine if you are eligible for the program
  • WILL I BE REQUIRED TO PAY FOR MY APPOINTMENT?
    • If you are on a CDM referral you are eligible for up to 5 allied health visits within the calendar year – it is up to you and your GP to decide which services you wish to allocate the referrals to
    • Patients are required to pay $65 for their consultations. Your Podiatrist will send the claim to Medicare and the patient will receive a rebate of $53.80 in their registered bank account. The total out of pocket expense is $11.20.
